Contexte : <\/strong>Les l\u00e9sions neurologiques subcliniques apr\u00e8s une arthroplastie invers\u00e9e de l’\u00e9paule sont fr\u00e9quentes, principalement celles qui touchent le nerf axillaire. L’un des principaux facteurs de risque rapport\u00e9s est l’allongement postop\u00e9ratoire du bras. Le but de cette \u00e9tude \u00e9tait d’\u00e9valuer la relation anatomique entre le nerf axillaire et les composants proth\u00e9tiques apr\u00e8s une arthroplastie invers\u00e9e de l’\u00e9paule. L’hypoth\u00e8se de l’\u00e9tude \u00e9tait que le d\u00e9passement inf\u00e9rieur de la gl\u00e9nosph\u00e8re par rapport \u00e0 la gl\u00e8ne pouvait pr\u00e9senter un risque pour ce nerf. <\/p>\n\n

Mat\u00e9riel et m\u00e9thodes : <\/strong>Onze sp\u00e9cimens frais d’\u00e9paule congel\u00e9e ont \u00e9t\u00e9 diss\u00e9qu\u00e9s apr\u00e8s avoir subi une arthroplastie invers\u00e9e de l’\u00e9paule par une approche deltopectorale classique.<\/p>\n\n

R\u00e9sultats : <\/strong>La distance moyenne entre le bord inf\u00e9rieur de la gl\u00e8ne et le bord inf\u00e9rieur de la gl\u00e9nosph\u00e8re \u00e9tait de 6,0\u00b14,3 mm (de 1,0 \u00e0 16,2 mm). Le nerf axillaire n’\u00e9tait jamais \u00e0 moins de 15 mm de la gl\u00e9nosph\u00e8re. La branche ant\u00e9rieure principale du nerf axillaire \u00e9tait en contact \u00e9troit avec la m\u00e9taphyse post\u00e9rieure ou l’implant proth\u00e9tique hum\u00e9ral. La distance moyenne entre le nerf et les implants hum\u00e9raux \u00e9tait de 5,2\u00b12,1mm (de 2,0 \u00e0 8,1mm). <\/p>\n\n

Conclusions : <\/strong>La proximit\u00e9 du nerf axillaire avec la m\u00e9taphyse post\u00e9rieure ou les implants hum\u00e9raux peut \u00eatre un facteur de risque de l\u00e9sion du nerf axillaire apr\u00e8s une arthroplastie invers\u00e9e de l’\u00e9paule.<\/p>\n\n
