Bien que la chirurgie soit couramment utilis\u00e9e pour traiter l’instabilit\u00e9 gl\u00e9no-hum\u00e9rale, il n’existe aucune preuve que ce traitement corrige efficacement la translation gl\u00e9no-hum\u00e9rale. Le but de cette \u00e9tude clinique prospective \u00e9tait d’analyser l’effet de la stabilisation chirurgicale sur la translation gl\u00e9no-hum\u00e9rale. La translation gl\u00e9no-hum\u00e9rale a \u00e9t\u00e9 \u00e9valu\u00e9e chez 11 patients en pr\u00e9op\u00e9ratoire et 1 an en postop\u00e9ratoire apr\u00e8s une stabilisation chirurgicale pour une instabilit\u00e9 ant\u00e9rieure de l’\u00e9paule. La translation a \u00e9t\u00e9 mesur\u00e9e \u00e0 l’aide de la capture optique du mouvement et de la tomographie assist\u00e9e par ordinateur. En pr\u00e9op\u00e9ratoire, la translation ant\u00e9rieure de l’\u00e9paule affect\u00e9e \u00e9tait plus importante que celle du c\u00f4t\u00e9 controlat\u00e9ral normal. Les diff\u00e9rences \u00e9taient significatives pour les mouvements de flexion et d’abduction (P < 0,001). En postop\u00e9ratoire, aucun patient n’a manifest\u00e9 d’appr\u00e9hension et tous les scores fonctionnels ont \u00e9t\u00e9 am\u00e9lior\u00e9s. Malgr\u00e9 l’absence d’appr\u00e9hension, la translation ant\u00e9rieure postop\u00e9ratoire des \u00e9paules stabilis\u00e9es chirurgicalement n’\u00e9tait pas significativement diff\u00e9rente des valeurs pr\u00e9op\u00e9ratoires. Si le traitement chirurgical de l’instabilit\u00e9 ant\u00e9rieure limite le risque de luxation, il ne semble pas restaurer la translation gl\u00e9no-hum\u00e9rale lors de l’amplitude fonctionnelle des mouvements. Cette micro-instabilit\u00e9 persistante peut expliquer les douleurs r\u00e9siduelles, l’appr\u00e9hension, l’incapacit\u00e9 \u00e0 reprendre une activit\u00e9 et m\u00eame l’\u00e9mergence d’une arthropathie de luxation observ\u00e9e chez certains patients.
