Le r\u00f4le de la morphologie osseuse de la grosse tub\u00e9rosit\u00e9 et de l&rsquo;acromion lat\u00e9ral a \u00e9t\u00e9 valid\u00e9 et peut \u00eatre mesur\u00e9 \u00e0 l&rsquo;aide de deux marqueurs radiographiques simples sur des vues ant\u00e9ropost\u00e9rieures r\u00e9elles : l&rsquo;angle de la grosse tub\u00e9rosit\u00e9 (GTA) et l&rsquo;angle critique de l&rsquo;\u00e9paule (CSA). Cependant, l&rsquo;interd\u00e9pendance de ces deux marqueurs reste inconnue, de m\u00eame que leur relation avec le niveau d&rsquo;activit\u00e9 professionnelle et sportive de l&rsquo;\u00e9paule. L&rsquo;objectif de cette \u00e9tude prospective et comparative \u00e9tait de d\u00e9crire la corr\u00e9lation entre le GTA et le CSA chez des patients souffrant de ruptures d\u00e9g\u00e9n\u00e9ratives de la coiffe des rotateurs.   <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Hypoth\u00e8se : <\/strong>Le GTA et le CSA sont des facteurs ind\u00e9pendants l&rsquo;un de l&rsquo;autre et des facteurs d\u00e9mographiques, tels que l&rsquo;\u00e2ge, la dominance, les sports ou les activit\u00e9s professionnelles.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Patients et m\u00e9thodes : <\/strong>Tous les patients se pr\u00e9sentant dans une clinique sp\u00e9cialis\u00e9e dans l&rsquo;\u00e9paule ont \u00e9t\u00e9 r\u00e9partis en deux groupes. Le premier \u00e9tait compos\u00e9 de patients pr\u00e9sentant une rupture d\u00e9g\u00e9n\u00e9rative symptomatique de la coiffe des rotateurs visible \u00e0 l&rsquo;IRM et le groupe de contr\u00f4le \u00e9tait compos\u00e9 de patients souffrant d&rsquo;autres probl\u00e8mes d&rsquo;\u00e9paule et n&rsquo;ayant pas d&rsquo;ant\u00e9c\u00e9dents ou de l\u00e9sion visible \u00e0 l&rsquo;imagerie de la coiffe des rotateurs. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>R\u00e9sultats : <\/strong>Il y avait 51 \u00e9paules chez 49 patients dans le groupe des ruptures de la coiffe des rotateurs (RCT) et 53 \u00e9paules chez 50 patients dans le groupe de contr\u00f4le. Les donn\u00e9es d\u00e9mographiques des patients \u00e9taient similaires dans les deux groupes. La GTA moyenne \u00e9tait de 72,1\u00b0\u00b13,7 (71,0-73,1) dans le groupe RCT et de 64,0\u00b0\u00b13,3 (63,1-64,9) dans le groupe t\u00e9moin (p&lt;0,001). La CSA moyenne \u00e9tait de 36,7\u00b0\u00b13,7 (35,7-37,8) dans le groupe RCT et de 32,1\u00b0\u00b13,7 (31,1-33,1) dans le groupe t\u00e9moin (p&lt;0.001). La somme des valeurs GTA et CSA sup\u00e9rieures \u00e0 103\u00b0 a multipli\u00e9 par 97 le risque de rupture de la coiffe des rotateurs (p&lt;0.001). Il n&rsquo;y avait pas de corr\u00e9lation entre la GTA et la CSA, ni entre la GTA ou la CSA et l&rsquo;\u00e2ge, le sexe, la taille de la d\u00e9chirure ou la dominance. Les patients ayant des niveaux d&rsquo;activit\u00e9 professionnelle et sportive diff\u00e9rents n&rsquo;avaient pas de valeurs GTA ou CSA significativement diff\u00e9rentes.      <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Conclusion : <\/strong>La GTA et la CSA sont des marqueurs radiologiques ind\u00e9pendants qui peuvent pr\u00e9dire de mani\u00e8re fiable la pr\u00e9sence d&rsquo;une rupture d\u00e9g\u00e9n\u00e9rative de la coiffe des rotateurs. Une somme des deux valeurs sup\u00e9rieure \u00e0 103\u00b0 multiplie par 97 les chances d&rsquo;avoir une rupture de la coiffe des rotateurs. Ces marqueurs ne sont pas corr\u00e9l\u00e9s aux facteurs d\u00e9mographiques ou environnementaux du patient, ce qui sugg\u00e8re que la variabilit\u00e9 de l&rsquo;acromion natif et la morphologie du trochiter peuvent \u00eatre des facteurs de risque individuels pour la rupture de la coiffe des rotateurs.  <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Niveau de preuve : <\/strong>II ;
